The Society to Improve Diagnosis in Medicine’s Patient Engagement Committee developed this toolkit with input from patients. The toolkit aims to help patients participate in the diagnostic process.
The toolkit is easy to understand and use. It has 4 parts:
- Prepare for My Appointment
- My Symptoms or Pain
- My Medicines
- After My Doctor Visit: What’s Next?
Patients fill out the first 3 parts of the toolkit before their health visit. They fill out the last part during (or soon after) the visit.
