Taking Charge of My Health Care Toolkit

The Oregon Self-Advocacy Coalition, the Oregon Office on Disability and Health, and the University Center of Excellence in Developmental Disabilities created this 2020 toolkit. The toolkit aims to help people with intellectual and developmental disabilities advocate for their health and healthcare. It’s also for their families/caregivers.

The toolkit has 12 modules that can be used in individual or group settings. Each module includes a workbook, worksheet, video, and PowerPoint slides. The toolkit also has a Leader’s Guide, handouts, and fact sheets.

The toolkit materials are easy to understand and use. They come in both English and Spanish.
