The Rural Health Information Hub created this toolkit to help rural organizations (e.g., healthcare providers, nonprofits, faith-based groups, businesses) develop community health worker (CHW) programs. It tells how to develop, implement, evaluate, and sustain rural CHW programs.
The toolkit has the following modules:
- Module 1: Introduction gives an overview of CHWs, their roles, and unique challenges rural communities face.
- Module 2: Program Models provides promising models for CHW programs.
- Module 3: Program Clearinghouse offers examples of CHW programs that have been implemented in rural communities.
- Module 4: Implementation discusses key issues to consider and address when implementing a rural CHW program.
- Module 5: Evaluation provides tools that can help with evaluating a CHW program.
- Module 6: Funding & Sustainability offers resources to help plan for funding and sustainability of a CHW program.
- Module 7: Dissemination includes ideas and resources for sharing findings from a CHW program.
