The Centers for Disease Control and Prevention’s Division for Heart Disease and Stroke Prevention developed this toolkit. The toolkit includes online courses, lessons learned, and best practices to help state health departments train and build capacity for CHWs in their communities. It also contains training and education materials for CHWs to use in their communities.
