The Toolkit to Engage High-Risk Patients in Safe Transitions Across Ambulatory Settings

The Agency for Healthcare Research and Quality developed this 2017 toolkit to help staff actively engage patients and their care partners to prevent errors during transitions of care.

The toolkit includes the following materials:

• Toolkit guide, which tells how to use the toolkit
• PowerPoint slide deck for team training
• Pre-Intervention Assessment, which helps team members identify safety gaps and areas for improvement
• Checklist to Prepare Patients for New Appointments, a set of best practices team members should follow with each patient
• Appointment Aide, a tool for patients to use at health visits
