The Conversation Project

This evidence-based initiative is run by the Institute for Healthcare Improvement. It seeks to help people talk about their wishes for care through the end of life, so those wishes can be understood and respected. It’s easy to understand and is meant for patients and their caregivers.

The initiative offers a collection of free guides on advance care planning. These cover the following topics:

  • Getting started
  • Assessing what matters to you
  • Being a healthcare proxy
  • Being prepared in the time of COVID-19
  • Caring for a child with a serious illness
  • Caring for people with Alzheimer’s and other dementias
  • Choosing a healthcare proxy
  • Talking with a healthcare team

The initiative also provides a wealth of other free resources on advance care planning. These include the following:

  • Resources for healthcare providers, faith leaders, and communities
  • Searchable map of conversation champions
  • Videos
  • Weekly blog posts

In addition, you can sign up to receive a newsletter and to participate in discussion groups for conversation champions.
