This quality improvement project evaluates embedding a Certified Health Education Specialist within a hematology/oncology inpatient unit to deliver tailored, health literacy–focused education. The specialist provided individualized instruction using Teach-Back, adapted materials to literacy levels, assessed social determinants of health, and supported discharge readiness. Early findings show improved patient understanding, better identification of SDOH needs, and stronger readiness for discharge. This targeted approach aims to reduce preventable 30-day readmissions and enhance outcomes for high-risk oncology patients.
Objectives:
-
After attending this session, attendees will be able to apply at least two health literacy–tailored communication strategies, including Teach-Back, to improve patient comprehension during discharge education.
-
After attending this session, attendees will be able to implement a consistent process for evaluating patient readiness for discharge and addressing identified gaps to strengthen the hospital-to-home transition.
-
After attending this session, attendees will be able to adapt educational materials to varying literacy levels using standardized techniques to enhance understanding and reduce preventable readmissions.
