On-Demand: From Visit to Understanding: Using AI to Support Personalized, Health-Literate After-Visit Care

After-visit summaries and follow-up instructions are key health literacy moments, yet many small and understaffed clinics rely on generic, hard-to-understand information. This session shows how AI can support organizational health literacy by helping care teams create clear, personalized after-visit guidance that patients can understand and act on. Participants will learn practical tools and workflows, including plain-language rewriting, prioritized “what to do next” checklists, teach-back prompts, and automated follow-up reminders. These solutions help clinics improve patient understanding, engagement, and follow-through while reducing staff burden and maintaining human oversight and trust.

Objectives:

  • Identify at least three high-risk after-visit and follow-up communication points where limited health literacy commonly affects patient understanding and engagement in small or understaffed clinics.

  • Apply a practical, step-by-step workflow for using AI to create clear, personalized after-visit summaries and follow-up guidance that patients can understand and act on.

  • Design one health-literate improvement to their own after-visit or discharge process using plain-language principles, teach-back prompts, and prioritized “next steps” to support patient engagement and follow-through and trust.
