Poster abstract: Over the last year, health educators at the health department partnered with Salt Lake County clinics to help improve their health literacy processes for patient education and communication. Our goal is to help implement and improve health literacy methods that will allow providers and patients to better communicate. These projects are completed through the PDSA cycle tool by gathering baseline data from the clinics, creating, and implementing new methods then adjusting the processes based on results. This program was created to help clinics implement long lasting health literate policies that will improve patient experience and overall health.
