Unlocking necessity and urgency for interventions, our study on Illinois; 1.3 million undiagnosed diabetic adults utilizes innovative CDCs interactive choropleth map to pinpoint priority counties for health literacy initiatives. We will demonstrate how these readily available resources/techniques identify targeted minority populations for initiatives. Our analyses link diabetes prevalence with the health literacy-related Social Vulnerability Index (SVI), highlighting Cook, Pulaski, Lawrence, Jefferson, and Winnebago counties as critical areas. Additionally, geospatial analyses using SVI indicators reveal elevated social vulnerability of English unproficiency in Cook and Winnebago counties. Our evidence-based strategies can enhance community health, fostering health equity and optimal outcomes.
