Webinar Description:

This webinar will demonstrate how the integration of Bluetooth home blood pressure monitors, apps, and an interactive digital platform (MyDataHelps by CareEvolution) into an evidence-based community wellness initiative can support the improvement of health and digital literacy and increase patient involvement in their own care.

Even before the 2020 COVID-19 pandemic, data on U.S. health disparities showed that persons living with chronic conditions, especially hypertension, were often the most vulnerable and the same persons at the highest mortality rates in the country. Among those with hypertension, only about half have their blood pressure under control, according the Center for Disease Control and Prevention. Each year, hypertension contributes to more than 400,000 deaths in the United States and contributes over $50 billion in direct medical costs and indirect costs due to lost productivity from premature death. Educating patients on the potential impacts of sustained uncontrolled blood pressure, helping them set realistic improvement goals with their primary provider, promoting the benefits of medication adherence, making positive changes to diet and physical activity, stress reduction and good sleep hygiene are just a few strategies that may facilitate hypertension control. The byproduct of this kind of effort can result in improvement to health and digital literacy, as well as improvement in patient/provider engagement.


Objectives:

By the conclusion of this webinar, participants will be able to:

  • Replicate this modestly budgeted, evidence-based, community-based SMBP wellness program focusing on the improvement of health/digital literacy.
  • Capitalize on the increased sense of awareness and support that ancillary primary care support can provide to foster the improvement in patient/provider engagement.
  • Integrate no to low cost digital platforms/apps into patient care or wellness programs to improve health and digital literacy and increase patient involvement in their own care.

Cheryl Thompson, MSPH

Cheryl J. Thompson is Co-Director of the Institute for the Advancement of Women’s Health (IAWH). She is also a Senior Public Health Analyst at the U.S. HHS Health Resource Services Administration, where she leads a team that supports the development of training and technical assistance to health centers on infectious disease and community health. Prior to her current position she served as a Public Health Advisor in the U.S. HHS Office on Women’s Health working on health topics as varied as Opioid misuse and addiction, overweight and obesity, intimate partner violence, trauma-informed care, heart health, HIV/AIDS, and female genital cutting.
